Residential Facilities And Intellectually And Developmentally Disabled Group Homes
StationMD is proud to have immense experience taking care of the Intellectually and Developmentally Disabled (IDD) and other special-needs populations in residential and group-home settings. We tailor our process to meet the needs of the patients in individual residential alternatives (IRAs), intermediate care facilities (ICFs), supportive apartments and day programs.
Although the details vary by client, the basics remain the same: When a medical issue arises, StationMD doctors are available immediately to perform a telemedicine exam. Our secure video-conferencing system, augmented by sophisticated diagnostic tools, allows our doctors to perform a thorough evaluation on the spot. We then work closely with staff, family and patients to implement a treatment plan. When necessary, prescriptions are sent, and a follow-up encounter is performed later in the day.
Documentation And Continuity Of Care
Formal documentation of the encounter is created and sent to the primary care provider and facility. When desired, the primary doctor is updated via secure text.
Complete Solution For Special Needs Population
As with other populations, StationMD has specialized medical protocols for IDD patients. Data analysis and feedback is given to facilities to create a complete quality-improvement solution. In our treatment of the IDD population, we’ve formulated robust data that demonstrates reductions in ER visits and hospital admissions, resulting in massive savings in health care dollars.
Family Involvement And ER Transfers
A patient’s family can be conferenced in to the video encounter (no matter where the family member is). This effectively lets them sit in on the examination and ask questions to the doctor directly and reassures them that concerns are being adequately addressed in an immediate and thorough fashion. On those occasions when transfer to the ER is unavoidable, the StationMD doctor communicates directly with the ER staff to streamline care and avoid unnecessary tests and admissions.
Home Care Services
Home care nursing services has become a pivotal tool in keeping patients at home and preventing unnecessary costs. Access to StationMD in this context can augment homecare in critical ways for these at-risk patients. In this process, if a visiting nurse or aid notes a concern, the first step is to contact a StationMD emergency physician for an immediate consultation.
Carrying a small, standard portable tablet device, a video conference encounter is initiated by the visiting nurse and an emergency medicine doctor evaluation is performed. This is done without the patient experiencing the hardship and risks of hospital transfer. Oftentimes an adjustment or treatment can be initiated by the StationMD doctor and transfer can be avoided.
If a hospital transfer is unavoidable, the StationMD physician communicates with the emergency department directly, thereby streamlining the work up, and decreasing the chances of admission. Better care is achieved while saving significant health care dollars.
