Skilled Nursing Facilities (SNFS)/Post-Acute Care Settings

StationMD’s program at nursing facilities improves both access to doctors and care, particularly during off-hours, when a physician is not on the premises. We have vast experience in taking care of patients in post-acute care and chronic care facilities. The StationMD process includes fine-tuned medical protocols, data analysis and feedback to facilities. In our treatment of this population, we’ve accrued robust data that demonstrates reductions in ER visits and hospital admissions resulting in massive savings in health care dollars.

Process

StationMD’s emergency medicine boarded physicians perform telemedicine consultations immediately upon request by the nursing facility staff. Our secure video-conferencing system is augmented by sophisticated diagnostic tools including an electronic stethoscope that transmits heart, breath and abdominal sounds to the doctor. Our doctors work closely with the nursing staff at the facility to provide the care the patient would otherwise get in the emergency room.

ER Transfers

On many occasions, conditions can be adequately addressed by the StationMD doctor without transfer. When transfer is unavoidable, the StationMD physician communicates directly with the emergency doctor at the hospital to streamline care, prevent unnecessary tests and decrease chances of admission.

Family Involvement

A patient’s family members can be conferenced in to the video encounter (no matter where the family member is). This effectively lets them sit in on the examination and ask questions to the doctor directly—and reassures them that concerns are being adequately addressed in an immediate and thorough fashion.
